Please keep in mind that the following items on the FDR application are fixed and part of the pricing grid, they cannot be adjusted on the FDR application:

- *Descriptors:* Please enter the 20 character descriptor in the *Customer Service Phone # field*. Please use the following example as a guideline: *Descriptor8885551212
Please note that there are no spaces or hyphens since each will be considered a character space ( a total of 20 characters).
*Application Submissions:* Please begin to include the SAQ Form with your application package to avoid the SAQ Non-Compliance Fee for the following quarter billing cycle. (see attachment for Form A and Form B)

*Acceptable Practices :*
Monthly auto ship – Opt in - includes open end recurring allowed
Discounted shipments – two for the price of one/three for the price of two, buy 1 get 1 free, etc.
Terms & Conditions should be properly disclosed and included on the payment page.
Pre-notification of billing in the open ended auto ship model should be encouraged strongly
· *Non Compliance Fee *
- Payment Card Industry Data Security Standards “PCI DSS” require all merchants accepting credit cards to have a Self-Assessment Questionnaire “SAQ” on file with their processor. In keeping with regulations, Our Processor will require this SAQ to be submitted in order to avoid a quarterly “Non Compliance Fee” of $30.00. Communications regarding this requirement has been included on the previous month processing statement as a reminder.
- Please see the attached SAQ forms and refer to the instructions below:
*Form A* = Card-not-present (e-commerce or mail/telephone-order) merchants, all cardholder data functions outsourced. This would never apply to face-to-face merchants.
*Form B* = Imprint-only merchants with no electronic cardholder data storage. This Validation Type applies to those merchants who take card information
manually and subsequently submit transactions for processing by phone via the Automated Response Unit (ARU).
*Form B* = Stand-alone terminal merchants, no electronic cardholder data storage. This SAQ should be used by those merchants who take any amount of face-to-face transactions and swipe or key card information into a terminal, software or gateway.
*QSA Compliant Verification*:
If your merchant's business has already completed a verification process with a Qualified Security Assessor (QSA), please submit a completed Report on Compliance or Report on Validation. You may submit the form to us via email, fax or postal mail using the contact information below. If you do
not have either of these documents, please provide the completed report or other acknowledgement provided to you from your QSA. Upon verification, your merchant account will be removed from the monthly non-compliance fee. With the QSA verification, you will not be required to complete the SAQ.

· *Quarterly PCI Compliance Program Fee*
- The PCI Compliance Program Fee has been reviewed and due to overwhelming merchant request; Our Processing bank will now apply this Program Fee on a Quarterly basis within the months of February, May, August and November. Merchants on the First Data Platform were previously assessed an annual fee in April. They will now be billed a Quarterly Program Fee beginning in May, in the amount of $39.95. This fee allows us to continue providing the high level of support with respect to the compliance standards set forth by the major card brands, PCI Security Council and other government agencies
- The PCI Compliance Program Fee as well as the Non Compliance Fee will be billed quarterly on both the First Data platform and Global platform customers. The Non Compliance Fee will only be billed to the customers who have* not* returned their SAQ or QSA certification forms.
- Notification has been sent to all customers on their monthly statement
